First off, there is a MUCH easier way to report copyright violations to Adsense. Rather than filing the notice, waiting for a few days and then getting hit with the request for proof, just file a DMCA notice with Adsense here:

http://www.google.com/adsense_dmca.html

One step and it's done, I've used it several times before.

However, if getting your work removed is more important, you can also file a DMCA notice with the host of the site, if the servers are located in America. All you need to do is find out who the host is and track down their DMCA contact information.

Personally, I have never had any luck with filing Adsense complaints via the method you describe. They either do not respond and fail to take action or simply write back and request more verification.

All in all, I've pretty much abandoned the use of that form.

Content theft has becoming a major issues that is happening these days especially in popular blogs where new bloggers just simply copy/paste the content to their blogs without prior copyright linkbacks.

Whilst it is troublesome, this is what is known as "He who alleges must prove". And prove, you must. Otherwise, the Google team might be a victim of false allegations and would have to spend much time and resources to investigate complaints.
